Course Curriculum

Having a foundation for effective case management is critical. This five-part series will cover solutions for many of the challenges faced by case managers today.

The series will first discuss key stakeholders and the integral role they play in the rounding process. Next, there will be a discussion on how to incorporate case management at all access points to your hospital or health system. This will be followed by the often-challenging subject of how to best collaborate with physicians, especially high-volume ones such as hospitalists and emergency department physicians. The series will then review how to remain forward looking in your practice. Finally, the series will conclude with a discussion of case management outcomes, including what they are, how to measure them, and how to report them.

Continuing Education Credits

The Georgia Hospital Association Research & Education Foundation is an approved provider of nursing home administrator credits as a professional trade association in long term care under Ga. Comp. R. & Regs. 393-13-.02, (1) (a).  Approved for 10 contact hours. 

By attending Beyond Basics of Case Management Boot Camp, Five Part Series offered by Georgia Hospital Association, participants may earn up to 10 ACHE Qualifying Education Hours toward initial certification or recertification of the Fellow of the American College of Healthcare Executives (FACHE) designation. Attendees who wish to have the hours applied toward ACHE Qualifying Education credit must self-report their participation by logging into their MyACHE account and selecting ACHE Qualifying Education Credit.
